IMPORTANT HEALTH COVERAGE TAX DOCUMENTS

Employees of Heirloom Living Centers, LLC may request a copy of their IRS Form 1095-C by contacting the Administrator of their Facility and making such a request.  IRS Form 1095-C is summary of offers of health coverage and enrollment in health coverage for full-time employees.

Notice:

If you are enrolled in our employer-sponsored, self-insured health coverage (and are a non-full-time employee or nonemployee), you can request a copy of your Form 1095-C tax statement.

This form reports your health coverage for tax purposes. You do not need to take action unless you want a copy.

To request your Form 1095-C:

  We will send your statement within 30 days of your request. For tax year 2025, requests must be fulfilled no later than the later of January 31, 2026, or 30 days after we receive your request.
  

Effective January 31, 2024, employers no longer need to automatically mail Form 1095-C to individuals. You may request yours using the contact information above.
